
Connecticut Medical Malpractice: When Hospitals Fail At Discharge Plans

One in five patients are discharged from a hospital to a post-acute care center, according to a report from the United Hospital Fund. This report cites that both patients and their families face significant challenges in this next phase of recovery.

Patients and families were found to face the following challenges:
- Less than 48 hours to prepare for movement to a care facility
- Federal law blocks hospital workers from providing recommendations on care
- Families and patients are not positioned to make the best decisions on care
- Quality of care varies
